Part A: Union Hospital Medical Check-up Package Terms& Conditions
甲部:仁安醫院體檢計劃條款及細則
Applicable specifically to health examination service packages. / 專門適用於醫療健康檢查服務計劃。
1. Service Provider / 服務供應商
· Health examination services are facilitated and conducted by Union Hospital. 健康檢查服務由仁安醫院提供及執行。
2. Identification and Verification / 身份證明與核實
· Upon arrival at the hospital, clients are required to present a valid form of identification including, but not limited to, a Hong Kong Identity Card or passport—alongside the official order confirmation letter for verification purposes. 客戶到達醫院後,須出示有效身份證明文件(包括但不限於香港身份證或護照)以及官方訂單確認信以作核實之用。
3. Appointment Scheduling / 預約安排
· Initial Contact: Union Hospital shall contact the client within two (2) business days of purchase to facilitate scheduling. 首次聯絡:仁安醫院將於購買後兩(2)個工作天內聯絡客戶以協助安排預約。
· Advance Notice: It is highly recommended that appointments be requested at least three (3) days in advance. 提前通知:強烈建議至少提前三(3)天提出預約申請。
· Weekend Bookings: Requests for Saturday appointments must be submitted a minimum of one (1) week prior to the desired date. 週末預約:星期六預約申請必須在所需日期至少一星期前提交。
· Post-Payment Booking: Clients may also contact Union Hospital directly via the provided hotline or WhatsApp once payment is confirmed and the confirmation email is received. 付款後預約:一旦確認付款並收到確認電郵,客戶亦可透過提供的熱線或WhatsApp直接聯絡仁安醫院。
4. Validity Period / 有效期
· Orders remain valid for three (3) months from the date of purchase. All services must be scheduled and completed within this time frame. Failure to utilize the service before the expiration date will result in the forfeiture of the order. 訂單自購買日起計三(3)個月內有效。所有服務必須在此期限內預約並完成。未能於到期日前使用服務將導致訂單失效及取消。
5. Medical-Related Amendments, Cancellations, and Refunds / 醫療相關更改、取消及退款
· Medical Exceptions: Requests for changes or cancellations due to medical necessity must be supported by a formal medical certificate or a physician's assessment. 醫療特殊情況:因醫療需要而提出的更改或取消申請,必須附有正式的醫生證明書或醫生的評估報告。
· Service Unavailability: If the service provider is unable to fulfill the order within the three-month validity period, a full refund will be issued, and the administration fee will be waived. 服務未能提供:若服務供應商未能於三個月有效期內履行訂單,將安排全額退款,並豁免行政費用。
· Rescheduling: Clients may modify appointment dates by contacting Union Hospital directly. There are no limits on the number of rescheduling requests within the three (3) months validity period from the date of purchase, and no additional fees will be incurred for these changes. 更改預約:客戶可直接聯絡仁安醫院以修改預約日期。在購買日起計的三(3)個月有效期內,更改預約的次數不設限制,且不會因此產生任何額外費用。
6. Inclement Weather / 惡劣天氣安排
· Services may be suspended or modified in the event of a Typhoon Signal No. 8 or above or a Black Rainstorm Warning. 如遇八號或以上颱風信號或黑色暴雨警告,服務可能會暫停或作出調整。
· Clients should consult the official website(www.healthx2pandas.com) for operational updates. No administration fees will apply to appointments rescheduled due to extreme weather conditions. 客戶應瀏覽官方網站 (www.healthx2pandas.com) 以獲取最新營業安排。因極端天氣條件而重新安排的預約將不收取任何行政費用。
7. Clinical Procedures and Results / 臨床程序及報告結果
· Preparation: Union Hospital will provide specific pre-examination instructions based on the selected package. 檢查準備:仁安醫院將根據所選的體檢計劃提供特定的檢查前指引。
· Reporting Time: Health examination reports are typically finalized within 7–10 working days post-checkup. Clients will be notified via telephone call or WhatsApp once the report is available. 報告時間:健康檢查報告通常在體檢後 7-10 個工作天內完成。報告完成後,將透過電話或WhatsApp通知客戶。
· Consultation: Union Hospital will arrange a follow-up appointment for the client on the day of the check-up for report collection ora results review with a physician, either in person or via telephone. 醫生面談:仁安醫院將在體檢當天為客戶安排後續預約,以便親身或透過電話領取報告或由醫生進行報告解說。
· Retention Policy: Reports must be collected within six (6) months of the examination date. Union Hospital reserves the right to dispose of unclaimed reports after this period without prior notice. 報告留存政策:報告必須在檢查日起六(6)個月內領取。仁安醫院保留在此期限後處理未領取報告的權利,恕不另行通知。
· Third-Party Collection: If a report is to be collected by a third party, they must present a formal authorization letter and a copy of the client’s HKID card. 第三方代領:如由第三方代領報告,代領人必須出示正式授權書及客戶的香港身份證副本。
8. Medical Disclaimer / 醫療免責聲明
· Medical Purpose: All tests conducted are intended for screening purposes only and do not constitute medical diagnosis or treatment. Clients are advised to consult their personal physicians regarding specific medical concerns. 醫療用途:所有進行的測試僅用於篩查目的,並不構成醫療診斷或治療。建議客戶就具體的醫療問題諮詢其個人醫生。

乙部:產品銷售條款及細則
Applicable to physical goods and productpurchases. / 適用於實體商品及產品購買。
1. Shipping & Delivery /送貨及物流安排
· Logistics Provider: All physical product orders are deliveredvia SF Express. 物流供應商:所有實體產品訂單均透過順豐速運 (SF Express) 進行派送。
· Delivery Timeline: Standard delivery is completed within 2 to 7business days from the date of order confirmation. 送貨時間:標準送貨服務將於訂單確認之日起 2 至 7 個工作天內完成。
2. Receipts andDocumentation / 收據及文件證明
· Issuance: 2 Pandas Limited will issue an official receipt uponformal request sent to cs@healthx2pandas.com. 收據發出:2 Pandas Limited 將在收到發送至 cs@healthx2pandas.com 的正式申請後發出官方收據。
· Consistency: The name appearing on the receipt must strictlymatch the name of the purchaser or intended recipient. 姓名一致性:收據上顯示的姓名必須與購買者或預期收件人的姓名完全一致。
3. Amendments,Cancellations, and Refunds / 更改、取消及退款
· Policy: No amendments or cancellations are permitted followingpayment confirmation. 政策:付款確認後,恕不允許任何更改或取消。
4. Disclaimers and Rights / 免責聲明與權利
· Right to Modify: 2 Pandas Limited reserves the right to amendthese product terms and conditions, adjust pricing, or invalidate offers at itssole discretion without prior notification. 修改權利:2 Pandas Limited 保留隨時自行決定修改此產品條款及細則、調整價格或取消優惠的權利,恕不另行通知。
